TOP 3 PREFERENCES: Fill out the registration form indicating your top 3 class time preferences in your respective age group for each participant you are signing up. You will be placed in a class that meets once a week for 8 weeks at the same day and time. We will do our best to get you in one of your top 2 class preferences but it is important to list 3 class preferences if at all possible in case a class is full, changed or removed from the schedule.

SIBLINGS: You can sign up all siblings on one form.

FRIEND REQUEST: If you are making friend requests, it is important to list the names in the note section of the form as well as listing the same class preferences.

PAYMENT: We recommend you have your credit card ready as you will not be able to go back later to complete payment. Payment is required to submit the registration form in order to reserve a spot in a class and to confirm your t-shirt order. In the event that we are unable to place you in one of your desired classes, we will refund you in full.

The cost of registration covers:
- full 8 week season (8 total classes)
- new Hot Shots Soccer t-shirt
- end of season celebratory medal!

WAIVER: A signed waiver must be completed for ALL participants. If you are signing up a new participant, indicate so at the completion of your registration form. You will be directed to the online waiver. If you have already played with us before and signed the electronic waiver, you do not have to sign it again.

Your form will be reviewed upon submission and class assignments will be sent out 1-2 weeks before class starts. Immediately after your submission is sent, you will receive 2 emails: a copy of your form + a Welcome email. *If you do not see it, please check your spam/junk/promotions folders. Sometimes our automated emails will get lost in there! If you still don't see it, let us know :)"
